Men searching for before-and-after results are usually asking a more useful question than the phrase suggests: what will actually be different? This page answers that aspect by aspect — what improves, what stays the same, and what an implant cannot do.

Erections are absent, unreliable, or not firm enough for penetration despite medication, injections or a vacuum device.

Rigidity is produced on demand by operating the device. It does not depend on arousal, blood flow or medication.

No. An implant is not a lengthening procedure. The cylinders are sized to your own anatomy, as measured during surgery. Some devices expand in length as well as girth when inflated, but that expansion is within the size implanted — it does not make the penis longer than it was. Anyone promising added length from an implant is overselling it.
Some men perceive the penis as slightly shorter after implant surgery. This is more likely where there was already shortening from Peyronie’s disease, previous surgery or long-standing erectile dysfunction with scarring. Discussing this openly before surgery is important, because expectation about size is one of the main drivers of dissatisfaction afterwards.
Penile skin sensation is carried by nerves that the implant does not involve, so sensation is generally unchanged. If your sensation was already reduced — by diabetes, nerve injury or previous surgery — an implant will not restore it. Temporary numbness or altered sensation around the incision during healing is common and usually settles.
No. Orgasm depends on nerve pathways rather than on rigidity, which is why many men can reach orgasm even without a usable erection. If you could reach orgasm before surgery, you can generally still do so afterwards.
The erection is firm and reliable but produced mechanically, so it is not identical to a natural erection. The glans does not become rigid with the device. Most men report it feels natural enough in use, and that operating the pump becomes routine, but the honest answer is that it is a different experience and it takes some getting used to.
With an inflatable device, nothing is visible under clothing when it is deflated. The pump can be felt in the scrotum if someone touches it, and a partner will usually know an implant is present. A malleable device keeps the penis firm at all times, which requires more care with clothing. No implant is detectable through normal social contact.
